Intake Forms

Please print and complete one of the intake forms below and bring it with you for your first visit.

The following form is only necessary for these prenatal complications: carrying more than one baby, high blood pressure, pre-eclampsia, history of preterm birth, liver or kidney dysfunction, placental problems. Please print it and have it signed by your prenatal healthcare provider before your first visit.


Policies

No-Stress Tipping Policy: In some spas, gratuities have become practically a requirement which I believe is not a fair business practice at all. Here is my promise to you: no matter if you tip or don't tip, you will always be treated with the same level of excellence and professionalism. I know that many variables contribute to whether or not a tip is offered, including: your budget, level of satisfaction, the type of service received, personal beliefs about tipping, etc. Gratuities, while graciously accepted, are completely optional at Tranquil Haven and never expected.

Payment Policy: Payment for your session is due at the end of your appointment. I gladly accept checks, cash, Mastercard, Discover and Visa. In the event that your check is returned from the bank, an additional $30 fee will be assessed to cover bank charges.

Punctuality: Please arrive on time to your appointment. If you arrive late to your scheduled appointment, we will do our best to help you still receive a relaxing session. However, we also need to maintain a specific schedule to meet the needs of all our clients. If you are late for an appointment, the massage will be given within the remaining time. If you are more than 30 minutes late, you will need to reschedule the appointment. In either case, you will be charged the full amount for the appointment.

Rescheduling: We request that if you need to cancel or reschedule an appointment that you give us at least 4 hours notice to allow sufficient time to change our schedule.

No Show Policy: If you do not show up for an appointment there are two options: 1. You pay a no-show fee (50% of the service(s) reserved) which will allow you to make future appointments with me or 2. You choose not to pay a no-show fee and I will no longer accept appointments with you.

Illness: It is our goal to maintain an environment of health and wellness. Therefore, to protect the health of all of our clients and staff, we ask that you call us at the earliest sign of illness to reschedule your appointment so that you can take the time to nurture yourself at home. Most massage therapy and spa treatments are contraindicated while sick. If you have a fever, severe cough, colored nasal discharge, or stomach ailment we reserve the right to refuse service.

Children: Children are always welcome in my studio. You are free to hold content/sleeping/nursing infants as you receive most services.
